Forms

Please make an appointment with your doctor to get a referral. This allows the doctor to ensure the specialist gets your most up-to-date medical information and is also our legal obligation under Medicare rules. Copies or renewals of referrals, certificates, faxing requests etc, incur a $10 administration fee to be paid before the request is fulfilled.

Prescriptions

Please make an appointment for a repeat prescription if you have not seen your doctor for three months or more. The doctors have a legal obligation to review your medical condition prior to writing a prescription. Most prescriptions provide sufficient medication to last until your condition needs to be reviewed by your doctor. If you request a copy of lost prescriptions (or repeats) without seeing the doctor a $10-$20 administration fee applies.

Referrals and Other Requests

All referrals require an appointment with a doctor; please make an appointment with your regular doctor. This allows the doctor to ensure the specialist gets your most up-to-date medical information which is our legal obligation under Medicare rules. Copies or renewals of referrals, certificates and other such requests incur a $10-$20 administration fee which must be paid before the request is fulfilled.

Transfer of Notes - Outgoing

We require written, signed consent to transfer your notes to another practice. Your new practice should have a request form that they can send to us, however, if they do not, we can provide a consent form to transfer your notes elsewhere. Please call the practice on 08 9418 3722 or visit the clinic during our opening hours and one of our receptionists will provide you with one. To cover administration costs, there will be a fee ranging from $25-$50 (a higher fee is attracted for paper note transfers and postage).

Transfer of Notes - Incoming

Written and signed consent is also required for us to request your notes from your previous practice. Please complete and return the form below (email is acceptable) to begin the transfer process. Alternatively, if you would prefer to complete a paper form please visit the clinic during our opening hours and one of our receptionists will provide you with one.
